This programme is for children who are at risk of delayed development because of his or her medical or physical needs.
Our team will work with you so that you and your child can be supported to access education, community and leisure opportunities - this may include:
- Occupational therapist
- Speech and language therapist
- Family support worker
- Early intervention teacher
How does it work?
You need to be referred to us by a paediatrician. Your child's needs are assessed by the team and we work out a home visiting plan with you.
At twelve months of age, we can discuss with you what their needs are likely to be. At eighteen months, we can make a referral with you to whichever programme or service will best be able to meet your child, family and whanau needs. This may be with a CCS Disability Action programme, or with another organisation.
